I usually just cross the road at Hugh Rae's. It can be busy though so allow extra time at rush hour. An alternate route would be up Castle banks and onward past the tilting ground. Very pleasant on a spring evening.

Depends what time you are travelling but there are two morning peak trains to London Bridge - then tube or bus from there (think the 59 bus goes pretty close). Otherwise train to Victoria then tube.
